ALLISON-DOAK SCHOLARSHIP PROGRAM Appalachian District of Civitan International INSTRUCTIONS
The scholarship is for those individuals who are mentally and/or physically challenged. Two scholarships of $1,000 each will be awarded. Awards are for one year. Candidates may reapply yearly. The application form may be printed and completed manually. Please make copies as needed. Place cursor over Allison-Doak Scholarship" link on left to show "2011 Application Form."
- The scholarships could be used for tuition or other expenses for further education at a college or vocational/technical school above the high school level and will be paid directly to the institution.
- Applicants may be students from high school or college and must live within the Appalachian District.
- The judges will evaluate applications with need being a priority. Applications will be judged on the basis of merit, community involvement, need (such as family income and number in family), recommendations, and overall presentation of the materials. Each category will be worth 20%. The judges will look at the total person and the potential for success.
A transcript of the applicant’s high school record must be submitted with the application. Should the applicant already be in college or vocational school, a current transcript must be included.
- Two letters of recommendation from responsible persons, not related to the applicant and who have known the applicant for at least two years must accompany the application. See application form for additional criteria.
- All applications must be neat and legible. They should be typed or neatly printed and three copies of the application, transcript, and letters of recommendation should be submitted.
- All applications must be submitted to the local Civitan Club no later than February 28, 2011. This time frame will allow for the winner to be recognized at the school awards ceremony. Please call your local club contacts to answer any questions you may have and to pick up the applications..
