All requests for assistance should be accompanied by an application filled out and signed by a doctor; rehab worker or community care access centre co-ordinator. All requests will addressed at the Board of Directors meeting from there request will be presented at the a regular business meeting; therefore please allow 4-6 weeks for all requests.
